We are contracted with and accept most PPO insurance plans. We will help you recover the most from your benefits. We will do everything we can to help you afford the treatment you need and want. Forms of payment accepted by the office are cash, check, Visa, Mastercard, Discover, American Express.
INSURANCE INFO
To make sure your dental visit goes smoothly, please follow these steps regarding your insurance:
- Verify your coverage is active. Call your insurance company to confirm this.
- Check if Dr. Samia Hardan (at the 1450 E. Boot Rd. location) is in your network. Your insurance company can tell you if she's a participating provider- she only practices at ONE location.
- Understand your insurance. While we're happy to assist, knowing your plan's details (like what's covered, co-pays, and yearly maximums) is ultimately your responsibility. Dental insurance isn't like medical insurance and often doesn't cover everything.
- Pre-authorization: For recommended treatments, we'll submit a pre-authorization to your insurance to estimate your coverage. Dr. Hardan does accept assignment of benefits, meaning, we only process your estimated co-pay at the time of your appointments, file to your insurance, and after they pay, we balance bill the patient if there is any balance left from the insurance not paying what they said they would.
- Out-of-network policies: If Dr. Hardan is out of your network, ask if you can choose your own dentist and if they will send payments directly to her (assign benefits).
- Payment: We will bill your insurance directly as a courtesy. You'll pay your estimated copay at your visit. If your insurance doesn't pay within 30 days, you'll be responsible for the remaining balance. We will review all estimates from insurance, and have you sign on every treatment plan. We keep a copy of everything for you in your chart.
- No assignment of benefits (out-of-network): If your plan doesn't pay out-of-network dentists directly, you'll need to pay the full amount at your visit, and your insurance will reimburse you directly.
- Medicare/Medicaid: We don't participate in Medicare or Medicaid. If you plan to use these as dental insurance, please call your plan to see if they reimburse out-of-network providers.
- Dr. Hardan is not a Medicare/Medicaid provider.
- In-network plans with Dr. Hardan include: Aetna PPO, United Concordia (various plans), Alliance (under Advantage), Sun Financial Administration, Alliance/Dental Network of America/Dentemax combination, BCBS of Texas, Highmark BCBS, Physicians Mutual, Equitable Dental, some Ameritas & Dental Network of America policies with U.C., others under Dentemax or Connection Dental, Sun Life and Sun Life Financial, Unum, Lincoln Financial Group, Principal Life Insurance Co.
- We are NOT in network with Philadelphia Federation of Teachers. You'll need to pay in full at the time of service, as their plan pays you directly.
- Delta Dental: Dr. Hardan participates at the Premier level. Please check with your plan to understand how this affects your benefits.
- Other in-network plans: Principal Insurance Co., Guardian, Ameritas, Connection Dental (GEHA), Unum (uses Connections), Humana PPO, TeamCare, Dominion Dental (Choice E PPO only), UMR, Renaissance Dental, Fidelio, Guardian Preferred.
- Out-of-network plans with Dr. Hardan include: All Medicare and Medicaid plans, BCBS, Cigna, Dental Health Alliance, MetLife, United Health Care. We have noticed some Medicare plans do reimburse, or pay (very minimal) on some of our routine patients' visits. Check with your dental coverage and ask them how you can get them to pay on your needed periodontal visits. Reach out to the office with any questions, we are happy to assist the best we can!
- Leased networks: If you have an out-of-network plan, ask if they use any "leased networks" that Dr. Hardan might be in. For example, some Cigna plans use other networks we participate in.
